Sports Physicals
The Family Health Centers, Inc. is offering sports physicals to middle and high school students athletes beginning Now through July 31, 2017. The cost is $20.00 per person.
Physicals for students entering grades 5th through 12th will be provided by FHC providers. Studetns mst bring their school's physical form completed and signed by a parent or legal guardian before an examination will be completed.
Individual appointments are not necessary. Walk-Ins Welcome!
